
Goal: Improve quality of decisions around colorectal cancer (CRC) testing in older adults through shared decision making.
TRAINING: 45-minute interactive SDM training session that provides key data for patients in this age range, a 3-step SDM model with scripts, and practice cases to apply the techniques. You can access the training from this link or via the QR code at the bottom of this page.
Epic/Cerner/IT: Non-interruptive EHR Alert when older patients (aged 76-85) are due or overdue for a CRC test at any visit that is non-urgent. The Alert is actionable, clinicians can order a test, send patient information, postpone alert, or add HM Modifier. It also has embedded links to resources, such as the Clinician Tipsheet and the Smartphrase (linked below).
VISIT: The EHR Alert & training prime the clinician to have an informed conversation with their patient about CRC testing options and supports documentation.
